Covid-19 Patient Guidance: Notifying Your Close Contact:
As case numbers continue to climb in WV, Health Departments will continue to contact all Covid-19 positive cases and those within their immediate household, however, we need your help to assist with preventing the spread of Covid-19 .
Please contact any additional close contacts that do not reside in your household.

 Online Requests for a Covid-19 Vaccination CardReplacement:
  You can request a Covid-19 vaccination card online via the WV DHHR 

  COVID-19 website.
  On the Vaccine page there is a button on the far right for 'Vaccination Card 

  Replacement'.   Click the button and follow the directions given to complete 

  an online form and a new card will be mailed to you.

At the recommendation of the FDA & CDC, we ARE currently providing an additional dose of Covid-19 Vaccine for those who are moderately to severe immunocompromised:
(Reference WV Health Advisory # 178).
- Active treatment for solid tumor and hematologic malignancies
Receipt of solid-organ transplant and taking immunosuppressive therapy
- Receipt of CAR-T-cell or hematopoietic stem cell transplant (within 2 years of transplantation or taking immunosuppression therapy)
- Moderate or severe primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich syndrome)
Advanced or untreated HIV infection
- Active treatment with high-dose corticosteroids (i.e., ≥20mg prednisone or equivalent per day), alkylating agents, antimetabolites, transplant-related immunosuppressive drugs, cancer chemotherapeutic agents classified as severely immunosuppressive, tumor-necrosis (TNF) blockers, and other biologic agents that are immunosuppressive or immunomodulatory
